Abstract |
Childhood obesity poses a major public health problem worldwide and there is strong evidence that foundation of cardiovascular diseases and type 2 diabetes lays early in life. Childhood obesity is also associated with psychological, respiratory, digestive, endocrine, and osteoarticular complications. There is therefore an urgent need for action to strengthen the treatment in obese children to pre-empt the development of obesity and co-morbidities in adult life.
The main purpose of this national multicentric project is to develop programs for the treatment of obese children and adolescents in Switzerland that will be reimbursed by health insurances, and to determine whether these interventions achieve the required objectives. This project aims also to observe individual changes over time and to identify predictors of changes and long-term maintenance. It will also examine the limitations in terms of organization or participation in group or modular therapeutic programs. Since childhood obesity therapy aims at long term lifestyle changes and improvement of family behaviours, a short-term reduction of adiposity as measured by body mass index is not a sufficient goal.
This is a longitudinal observational prospective study that will incorporate all overweight children and adolescents treated in group or modular programs from 2008 to 2012 in Switzerland and patients that are not eligible/able to participate in such programs and having no therapy and/or minimal medical treatment. The effectiveness and efficacy of group or modular programs have been demonstrated previously, but most obese children and adolescents have complex medical, behavioural and psycho-social problems that limit the participation in group therapies and require specific individual treatment. A Swiss network for childhood obesity therapy will be developed and an electronic database system will be implemented to guarantee quality control and benchmarking for the participating therapeutic centres. Primary outcome measures will be recorded at baseline, 6 months, 1 and 2 years after the start of therapy and will include: body mass index, waist circumference, body fat mass (in some centres), blood pressure, biological markers of the metabolic syndrome, physical activity and fitness, nutrition, psychosocial factors and health-related quality of life. These measures are essential to assess the degree of co-morbidities, lifestyle and psychosocial risk factors and to follow the evolution of the child. |
