Abstract Separation anxiety disorder is the earliest and most common anxiety disorder in childhood. Recent research indicates that anxiety disorders in childhood, and in particular Separation Anxiety Disorder (SAD), increase the risk of mental disorders in adulthood significantly. Despite these results, research on childhood anxiety disorders is still neglected compared to the vast research activity on adult anxiety disorders. In fact, there is almost no etiological and treatment research in young children. This contrasts sharply with the claim that children in particular should receive safe and evidence-based treatments. Thus, there is an urgent need for research in this age group. The present research program includes an etiological and a treatment study in children with SAD. One hundred and seven children with SAD, 62 children with other anxiety disorders and 46 children without mental disorders ages 5 to 14 underwent a comprehensive cognitive and family assessment. Further, all children with SAD completed a randomized controlled trial comparing a newly developed SAD-specific family-based treatment either with a well-established global child-based CBT program for childhood anxiety disorders or a waitlist condition. Initial analyses of our data found evidence for several major etiological hypotheses. Moreover, results of the treatment study indicate very good outcomes for both the specific family based CBT program and the global child-based CBT program in treating SAD.
